Your Rights

Access your records. You may request to inspect or receive a paper or electronic copy of the health and billing records we maintain about you. We generally respond within 30 days. If a permitted extension is necessary, we will explain the reason and completion date in writing. A reasonable, cost-based copying fee may apply.

Request a correction. You may ask us to amend information you believe is inaccurate or incomplete. We generally respond within 60 days. If a permitted extension is necessary, we will notify you in writing. If we deny your request, we will explain why in writing and explain your right to submit a statement of disagreement.

Request confidential communications. You may ask us to contact you by a particular method or at a different address. We will accommodate reasonable requests.

Request limits on information use or disclosure. You may request restrictions on information used or shared for treatment, payment, or pharmacy operations. We generally do not have to agree. However, when you pay for an item or service entirely out of pocket and request that we not disclose the related information to your health plan for payment or operations, we must honor that request unless disclosure is required by law.

Receive an accounting of disclosures. You may request a list of certain disclosures made during the preceding six years. Some disclosures, including most disclosures for treatment, payment, and operations, are excluded. One accounting in a 12-month period is free. We may charge a reasonable fee for additional requests after notifying you of the fee and giving you an opportunity to withdraw or modify your request.

Receive a paper notice. You may request a paper copy of this notice at any time, even if you previously agreed to electronic delivery.

Use a personal representative. A person legally authorized to act for you may exercise your rights, subject to applicable law. We will verify that authority.

How We Use and Share Your Information

We may use or disclose health information without your written authorization for:

  • Treatment: Filling prescriptions, checking medication interactions, coordinating care, and communicating with your prescriber or other treating providers.
  • Payment: Processing insurance claims, determining coverage, and collecting payment.
  • Health care operations: Managing pharmacy services, reviewing quality and safety, training staff, and conducting permitted administrative activities.

We may share information with service providers acting on our behalf under agreements requiring them to safeguard it.

When applicable legal conditions are satisfied, we may also use or disclose information for public health activities, medication recalls and adverse-event reporting, reporting abuse or neglect, preventing serious threats to health or safety, health oversight, research, workers’ compensation, certain law enforcement and government activities, organ donation, and duties of coroners, medical examiners, or funeral directors.

We may disclose information when required by law or in response to qualifying court orders, subpoenas, or other legal process. These disclosures remain subject to applicable privacy protections.

Your Choices and Written Authorization

We may share relevant information with family members, caregivers, or others involved in your care or payment when you agree or have an opportunity to object. If you cannot express a preference, we may share relevant information when professional judgment indicates it is in your best interest. Similar rules apply to disaster relief disclosures.

Uses and disclosures requiring authorization—including most uses of psychotherapy notes, marketing requiring authorization, and the sale of protected health information—will occur only with your written authorization.

Other uses or disclosures not permitted by law or described in this notice also require authorization. You may revoke an authorization in writing. Revocation does not undo actions already taken in reliance on it.

If we contact you for fundraising, you may opt out of further fundraising communications.

Additional Protections for Certain Records

We follow applicable laws that provide greater protection for particular types of health information.

If we receive substance use disorder patient records protected by 42 CFR Part 2, we will observe the additional restrictions that apply to those records. We will not use or disclose them in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a qualifying court order and subpoena.

Before using protected Part 2 records for fundraising communications, we will provide a clear advance opportunity for you to choose whether to receive those communications.

Our Responsibilities

We are required by law to maintain the privacy of your protected health information, provide this notice of our legal duties and privacy practices, and follow the notice currently in effect. We must notify you when a breach of unsecured protected health information requires notification under applicable law.
